健康 & 健康中心

Springfield Technical Community 大学 is committed to supporting student healthcare needs. A 自我保健的房间 is available to students and employees. It may be used for lactation, 伤口护理, self-administration of medications, meditation and other reflective practices, and nearly any other lawful healthcare need. The 自我保健的房间 is located within the 健康 and 健康中心, 爱尔兰共和军H. Rubenzahl 学生 Learning Commons (B19), Room 177. The room is available on a first-come/first-served basis. Room use is limited to 30-minute intervals, is subject to the 学生 Code of Conduct and all other applicable 大学 政策.

学生s enrolled in the School of 健康 and Patient Simulation (护理, 牙科, 医学成像, Medical Assisting, Rehabilitation Therapies, 等.), Early Childhood Education, non-credit healthcare programs (CNA, EMT, 放血), Behavioral Sciences, or the 健康科学 program.

Each of these programs has a checklist of required forms and health records needed for newly enrolled students and are provided via the PDF links below.

Where can a student get these immunizations if he/she has no proof?

If a student needs to be re-immunized because of age or inability to locate his/her record, there are many locations and clinics to obtain vaccinations, some at a reduced rate. Tuberculosis testing, seasonal flu vaccinations, and physical exams are by appointment only.

Request for Copy of Vaccination Records

You may download the Authorization To Release or Obtain 健康 Records Form (PDF) and return it to the 健康 and 健康中心 office in person, by mail, or 通过传真. Alternatively, you may complete the Authorization To Release or Obtain 健康 Records eForm (十大彩票平台 Account login required). Please allow 1 - 2 business days for the request to be processed.

学生运动员 are required to submit vaccination records (form listing requirements attached below).

• immunity to tetanus (vaccination within the last 10 years)
• immunity to pertussis (single dose of pertussis containing vaccine after age 18)
• immunity to measles (two live-vaccine doses or positive titer results)
• immunity to mumps (two live-vaccine doses or positive titer results)
• immunity to rubella (two live-vaccine doses or positive titer results)
• immunity to hepatitis B (three vaccine doses or positive titer results)
• immunity to chickenpox, known as varicella (two vaccine doses, positive titer results, or documented history of disease)

All Athletes must complete a one time Athletic Sickle Cell Trait NJCAA Form (PDF)
